      Adverse events following immunisation (AEFI) in the National Vaccination Programme of the Netherlands (RVP) have been monitored through an enhanced passive surveillance system by RIVM since 1962. From 1984 until 2003 evaluation has been done in close collaboration with the Health Council. An RIVM expert panel continued the reassessment of selected adverse events from 2004 onwards. Reports were received mainly from Child Health Care professionals, primarily by telephone through the operating service for information and advice on vaccines and vaccinations. Further data have been obtained, if necessary, from parents, general practitioners, paediatricians and other professionals. After supplementation and verification of data a (working) diagnosis is made and causality assessed. In this annual report on 2005 an overview of all reported AEFI is presented with classification according to case definitions and causality. Trend analysis, reporting bias, background rates of specific events and possible pathophysiology of symptoms are discussed. On a total of over 1.4 million vaccination dates 1036 AEFI were reported. Of these, 5 (0.5%) were unclassifiable because of insufficient information. In 73% (752) of the classifiable events a possible causal relation with vaccination was established. These concerned major adverse reactions in 47% and minor adverse reactions in 53% of the reports. Of the reported adverse events 27% (279) were considered chance occurrences. Compared to 2004 and 2003 there was a decrease in number of reports with 52% and 25% respectively. The huge increase is 2004 was caused by repeated media attention about the safety of the formerly used whole cell pertussis vaccine. The decrease in 2005 was caused by the use of a new, acellular DPTP-Hib vaccine. The Netherlands Vaccination Programme has a very favourable risk balance.

Reports are received mainly from Child Health Care professionals primarily by telephone through the operating vaccine information and advisory service. Further data are obtained, if necessary, from parents, general practitioners, paediatricians etceteras. After supplementation and verification of data a (working) diagnosis is made and causality assessed. In this annual report on 2003 an overview of all reported AEFI is presented with classification according to case definitions and causality. Trend analysis, reporting bias, background rates of specific events and possible pathophysiology of symptoms are discussed. On a total of over 1.5 million vaccinations 1374 AEFI were reported Of these 16 (1.2%) were unclassifiable because of missing information. In 78% (1060) of the classifiable events a possible causal relation with vaccination was established and in 22% (298) the events were judged to be coincidental. Compared to 2002 there were no relevant changes in numbers, causality or severity of reported adverse events. The Netherlands Vaccination Programme has a very favourable risk balance.

Of these, 9 (0.4%) were unclassifiable because of insufficient information. In 83% (1765) of the classifiable events a possible causal relation with vaccination was established. These concerned major adverse reactions in 56% and minor adverse reactions in 44% of reports. In 17% (367) of the reports the adverse events were considered chance occurrences. Compared to 2003 there was an increase in number of reports of 56%. This increase was caused by adverse publicity on the safety of the DPTP vaccine. However, this increased attention unveiled no unexpected severe or new adverse events. This adverse publicity started in the first week of 2004 and was immediately picked up by the system. Despite the increase in number of reports the Netherlands Vaccination Programme has a very favourable risk balance.

      Het Rijksvaccinatieprogramma (RVP) wordt sinds 1962 intensief bewaakt. De meldgraad van vermoede bijwerkingen is hoog met een goede meldbereidheid van de consultatiebureaus. Er is een relatief beperkte onderrapportage. Van de meldingen werd 78% als bijwerking van de vaccinaties beschouwd. Het ging hierbij om 1060 ziektebeelden, waarvan 56% heftiger verschijnselen betrof en 44% mildere klachten. Dit aantal bijwerkingen moet in relatie worden gezien tot de 1,5 miljoen vaccinaties en de bijna 7 miljoen vaccincomponenten die daarbij worden toegediend. Er zijn 1374 meldingen ontvangen in 2003. Het merendeel van de gemelde bijwerkingen betrof de bekende meer gewone klachten, zoals koorts, huilen, bleekheid en lokale verschijnselen. Hangerigheid kan zich op jonge leeftijd op vele manieren uiten. Een klein deel had heftiger verschijnselen zoals zeer hoge koorts, langdurig heftig huilen of forse lokale klachten. Zes kinderen hadden een abces (etterbuil) op de prikplek gekregen. Collaps (wegraking) kwam bij 210 meldingen voor en de zogenoemde verkleurde benen bij 134 kinderen, vooral na de eerste prik. Stuipen, vooral bij de eenjarigen en meestal gepaard met koorts, kwamen bij 70 kinderen voor en werden voor tweederde als bijwerking beschouwd; bij de overige kinderen had de stuip een andere oorzaak. Niet goed te duiden incidenten worden onder "atypische aanvallen" gerubriceerd. In bijna driekwart van deze gevallen was er een mogelijke relatie met de vaccinatie; het gaat dan om episodes met rillerigheid, schrikschokken en gespannenheid of juist een hele slappe houding. Hoewel al deze bijwerkingen omstanders erg kunnen laten schrikken zijn ze medisch gezien niet gevaarlijk en laten ze geen restverschijnselen na. Er zijn geen gevallen van hersenontsteking gemeld in 2003 en evenmin bedreigende allergische reacties. De ernstige infecties die werden gemeld hadden geen relatie met de vaccinaties en datzelfde gold voor de gemelde kinderen met epilepsie of suikerziekte. Het ging hierbij om een toevallige samenloop van gebeurtenissen. Bij de drie gemelde overleden kinderen is het overlijden niet door de vaccinaties veroorzaakt Een bijzondere en zeldzame mogelijke bijwerking die in 2003 wat vaker dan in andere jaren werd gemeld, was een tijdelijk tekort aan bloedplaatjes (ITP), met name na de vaccinatie tegen bof, mazelen en rodehond (BMR). De reden hiervoor is een studie die het RIVM samen met de kinderartsen hiernaar uitvoert. Hoewel heftige bijwerkingen na de RVP vaccinaties optreden, zijn ze voorbijgaand en leiden ze niet tot blijvende gevolgen. De grote gezondheidswinst die het RVP oplevert, weegt op tegen de bijwerkingen .

      De bijwerkingenbewaking van het Rijksvaccinatieprogramma over 2004 liet een duidelijke toename zien van het aantal meldingen met 56%. Deze toename betrof vooral de mildere en heftiger gewone bekende bijwerkingen als huilen en koorts. De toename in het aantal meldingen is toe te schrijven aan de onrust in de media over de veiligheid van de vaccinaties. Er zijn echter geen nieuwe, onverwachte of ernstige bijwerkingen aan het licht gekomen. In 2004 zijn in het totaal 2141 meldingen ontvangen. Hiervan werd 83% als bijwerking van de vaccinaties beschouwd. Het aantal bijwerkingen moet in relatie worden gezien tot de 1,5 miljoen vaccinaties en de bijna 7 miljoen vaccincomponenten die daarbij worden toegediend. De ernstige infecties die werden gemeld hadden geen relatie met de vaccinaties en datzelfde gold voor de gemelde kinderen met epilepsie of suikerziekte. Het ging hierbij om een toevallige samenloop van gebeurtenissen. Bij de vier gemelde overleden kinderen is het overlijden niet door de vaccinaties veroorzaakt. De gestimuleerde passieve veiligheidsbewaking is een goed en gevoelig instrument om signalen over mogelijke bijwerkingen op te pikken; het systeem laat tevens follow-up onderzoek toe. Hoewel heftige bijwerkingen na de RVP vaccinaties optreden, zijn ze voorbijgaand en leiden ze niet tot blijvende gevolgen. De grote gezondheidswinst die het RVP oplevert, weegt op tegen de bijwerkingen.

      From October 2000 the Dutch influenza surveillance is temporarily expanded to a case-control study on acute respiratory infections (ARI) in general practitioner (GP) patients: the ARI-EL study. The aim is gaining insight into 1) the incidence and aetiology of ARI, 2) risk factors for contracting ARI and 3) health-care demand and burden of illness due to ARI. GPs from the Netherlands Institute for Health Services Research (NIVEL) register all patients consulting them for ARI. Weekly, GPs sample one case, defined as one of the patients consulting them with complaints of ARI, and one control, defined as a patient consulting them without complaints of ARI. The samples are analysed at the National Institute of Public Health and the Environment (RIVM) in Bilthoven for respiratory viruses by culture and PCR, and for respiratory bacteria by culture at the Regional Laboratory of Public Health in Tilburg. Participating patients filled in a questionnaire at home. The background and methods of the ARI-EL study are presented in this interim report, along with some of the preliminary results of the first study year. In the controls, statistically significant fewer viruses were detected; in 53% of samples of the cases viruses were detected, compared to 19% of the samples from the controls. Influenza virus and rhinovirus were detected significantly more often in the cases than in the controls. Bacteria were detected in the same measure in the cases and controls. Only beta-haemolytic streptococci group A were significantly more often detected in the cases. In 31% of the cases and in 67% of the controls no micro-organisms other than the commensal flora were detected. The study has been found to be feasible, despite the burden for the GPs. It will continue until at least September 2002 to enable us to collect sufficient data for statistical analyses, and consequently to summon sufficient power to draw conclusions. The loss of data by applying the precise case and control definitions plus the fact that winter 2000/2001 was an extremely calm influenza season emphasise the need for data collection during a sufficiently long period. Then this exceptional study will provide information on the yearly and seasonal incidence of ARI and associated pathogens, the burden of illness, the health-care demand and the risk factors for contracting ARI.

      Vanaf oktober 2000 is de Nederlandse influenza-surveillance tijdelijk uitgebreid tot een case-controle studie naar acute respiratoire infecties (ARI) bij huisartspatienten: de ARI-EL studie. Doel is inzicht verkrijgen in de incidentie en etiologie van ARI, risicofactoren voor ARI en in de zorgvraag en ziektelast tengevolge van ARI. Huisartsen van het Nederlands Instituut voor Onderzoek van de Gezondheidszorg (NIVEL) registreren wekelijks het aantal consulten voor ARI en bemonsteren maximaal een case en een controle per week. De monsters worden onderzocht op respiratoire pathogenen op het Rijksinstituut voor de Volksgezondheid en Milieu (RIVM) en op het Streeklaboratorium voor de Volksgezondheid Tilburg met kweek en PCR. De deelnemende patienten vullen thuis een vragenlijst in. Bij cases werd significant vaker een virus aangetoond (bij 53 % van de cases versus 19% van de controles). Influenzavirus en rhinovirus werden significant vaker aangetoond in cases. Het percentage van de monsters waarin een bacterie werd aangetoond verschilde niet significant tussen cases en controles. Alleen beta-haemolytische streptokokken groep A werden significant vaker aangetoond bij cases. Bij 31% van de cases en bij 67% van de controles werd geen ziekteverwekker aangetoond Het blijkt dat de studie haalbaar is, maar wel redelijk belastend voor de huisartsen. De ARI-EL studie loopt door tot en met minimaal september 2002. Dit is nodig om voldoende gegevens te verzamelen voor statistische analyse met voldoende power om conclusies te kunnen trekken. Het verlies van gegevens door strikte toepassing van de case- en controledefinities en het feit dat winter 2000/2001 een uitzonderlijk rustig influenzaseizoen is geweest, benadrukken het belang van voldoende lange gegevensverzameling. Dan zal deze unieke studie informatie opleveren over de jaar- en seizoensincidentie van ARI en daarmee samenhangende pathogenen, ziektelast, zorgvraag en determinanten bij huisartspatienten.<br>

      Executive summary Avian flu epidemic 2003: public health consequences.Risk factors, health, well-being, health care needs and preventive measures during the H7N7 avian flu outbreak control in the Netherlands.An estimated thousand people, possibly more have been infected with avian flu during the outbreak in the Netherlands in 2003. One third of the poultry farmers whose holdings were cleared reported stress reactions, fatigue and depressive symptoms. The large spread of the virus underscores the importance of the measures to prevent poultry-to-human transmission in people handling infected poultry. The possible uncertainty, stress and anxiety associated with the avian flu control demand specific health care attention. A total of 453 people reported with health complaints, predominantly conjunctivitis. Antibodies were found in 59% of infected poultry workers' family members. Of the 500 tested persons who had handled infected poultry, about 50% showed an antibody response. The poultry farmers and workers complied insufficiently with the preventive measures. The antiviral drug oseltamivir protected against infection, whereas mouth and nose masks did not. The attention for support, information and respectful treatment of poultry farmers and workers during the clearances worked quite well. Externally hired veterinarians experienced their activities as emotionally aggravating more often than other professionals. About a quarter of the poultry farmers worried about the survival of their holding and the sector as a whole; 16% felt a need for additional support, help or health care because of the avian flu. They consulted agricultural care providers and family doctors, and less frequently mental health care providers.

    • Een bevolkingsonderzoek in vier regio&apos;s in Nederland naar de incidentie en ziektelast van gastro-enteritis en Campylobacter- en Salmonella-infecties

      Om te komen tot een schatting van de incidentie van gastro-enteritis, Campylobacter en Salmonella-infectie in de algemene bevolking is in 1991 een populatiestudie uitgevoerd in een aantal gemeenten verspreid over Nederland. Tevens diende deze studie een schatting te geven van het deel van de gastro-enteritis-episodes dat gedekt wordt door onderzoek binnen huisartsenpopulaties. Aan het onderzoek hebben 2206 personen deelgenomen (35% respons), die een persoontijd bijdroegen van 660 persoonjaren. De gestandaardiseerde incidentie van gastro-enteritis was 447 eerste episodes per 1000 persoonjaren. De gestandaardiseerde incidentie van Campylobacter was 86 eerste infecties per 1000 persoonjaren ; van Salmonella was dit 35 eerste infecties per 1000 persoonjaren. Uit 4,5% van de faecesmonsters van personen met gastro-enteritis werd Campylobacter geisoleerd, uit 1,6% Salmonella. 91% van de gastro-enteritis-episodes met Campylobacter was toe te schrijven aan deze infectie. Voor 22% van de eerste episodes werd een huisarts geconsulteerd. Extrapolatie naar de Nederlandse bevolking gaf een geschat aantal van 7,0 miljoen episodes van gastro-enteritis per jaar. Jaarlijks zouden in Nederland 1,5 miljoen episodes van gastro-enteritis gepaard gaan met gemiddeld 1,5 consultaties van een huisarts, 3,8 miljoen episodes zouden gepaard gaan met geneesmiddelengebruik en 12000 jaar zouden worden verzuimd van school of werk. De incidentie van gastro-enteritis-episodes waarvoor een huisarts geconsulteerd werd ligt tienmaal zo hoog als in de huisartsenpeilstation-onderzoeken. Dit leidt tot de conclusie dat resultaten van huisartsenpeilstationonderzoek niet direct naar de populatie te extrapoleren zijn, op basis van het percentage personen met gastro-enteritis dat de huisarts bezoekt. De extrapolatiefactor kan slechts worden achterhaald indien de data van een populatiestudie direct kunnen worden gekoppeld aan de data van een huisartsenpeilstationstudie.<br>

      The European Antimicrobial Resistance Surveillance System (EARSS) is an international initiative funded by the Director General for Health and Consumer Protection (DG SANCO) of the European Commission and the Dutch Ministry of Health, Welfare and Sports. It maintains a comprehensive surveillance and information system that links national networks by providing comparable and validated data on the prevalence and spread of major invasive bacteria with clinically and epidemiologically relevant antimicrobial resistance in Europe. EARSS collects routinely generated antimicrobial susceptibility (AST) data, provides spatial trend analyses and makes timely feedback available via an interactive website at www.rivm.nl/earss. Routine data for major indicator pathogens (Streptococcus pneumoniae, Staphylococcus aureus, Enterococcus faecalis, Enterococcus faecium, Escherichia coli, Klebsiella pneumonia and Pseudomonas aeruginosa) are regularly submitted by over 900 laboratories serving around 1400 hospitals in 32 European countries. By the end of 2005 two new countries joined the EARSS initiative, Lithuania and Turkey. Based on a previous laboratory/hospital questionnaire, the overall hospital catchment population of the EARSS network is estimated to include over 100 million inhabitants in the European region, with national coverage rates that ranged between 20-100% for individual countries. In 2005, information on the laboratory demands for external quality assessment (EQA) was collected by questionnaire. Several countries do not have formal agreements on national or international quality assessment schemes in place. Among the international providers of EQA the British UK-NEQAS scheme was most frequently named by countries. Alternatively, different national schemes are in place, either alone or in combination with one of the international programs. Importantly, the majority of laboratories that participate in EARSS utilise some type of EQA, demonstrating their commitment to diagnostic accuracy. In Europe the proportion of antibiotic resistant S. pneumoniae keeps changing with decreasing penicillin-resistance in some highly endemic countries and with continuous loss of susceptibility against penicillin and erythromycin in others. The main resistance phenotypes in pneumococci are confined to few serogroups, all of which are included in the currently promoted conjugate vaccines. This suggests that vaccination, especially in young children, may represent an effective additional means of controlling antibiotic resistance in pneumococcal disease in Europe. The increase of MRSA is consistent throughout Europe and includes countries with high, medium and low baseline MRSA endemicity. At the same time it appears that the MRSA pandemic is not an irreversible secular trend as two European countries (Slovenia and France) succeeded in constantly reducing the proportion of MRSA among S. aureus blood stream infections over the past five or six years. The speed with which fluoroquinolones loose their activity against E. coli is next to no other compound pathogen combination in the EARSS database. Combined resistance is a frequent occurrence, with co-resistance to three antimicrobial classes including third generation cephalosporins already among the four most common resistance patterns encountered in invasive E. coli in Europe, and undeniably these resistance traits are on the increase as well. In K. pneumoniae a high prevalence of resistant strains to third generation cephalosporins, fluoroquinolones and aminoglycosides becomes evident in Eastern and Southeastern Europe. Combined resistance is the dominant threat imposed by invasive P. aeruginosa. Our data suggest that the same geographical gradient exists for all gramnegative pathogens and shows that lower resistance prevails in the Northwest with increasing resistance towards the Southeast of Europe. It appears that the overall threat imposed on European communities by the increasing loss of antimicrobial effectiveness continues unabated with the same speed as has been previously described by our network. This is shown most convincingly among the pathogens that are frequently transmitted in health care settings (MRSA and VRE) and for antimicrobial compounds that are available for oral administration and hence preferred in ambulatory care (aminopenicillins, macrolides, and fluoroquinolones). The growing availability of third-line antimicrobial drugs as oral formulations is in this context a matter of concern and underscores the need of locally or nationally advised prescribing practices for both ambulatory and hospital-based care.

      In a porspective prevalence and incidence survey in The Netherlands in 1999 antimicrobial susceptibility data on invasive Streptococcus pneumoniae and Staphylococcus aureus infections were collected sithin the framework of European Antomicrobial Resistance Surveillance System (EARSS). The EARSS project covered approximately 40% of the Dutch population (extramural) and 40% of the total number of patient-days (intramural). Penicillin resistance in S. pneumoniae was minimal; only 9 of 767 (1,2%) isolates were non-susceptible. Resistance to oxacillin in S. aureus was low, only (0,3%) isolates were MRSA (mecA positive). The incidence of invasive S. pneumoniae was 117 cases/1.000.000 person-years; the incidence of invasive penicillin non-susceptible S. pneumoniae was 1 case/1.000.000 person-years. The incidence of invasive S. aureus infections was 0.25 cases/1000 patient-days; the incidence of invasive MRSA infections was 0.0006 cases/1000 patient-days. It may be concluded that resistance to antibiotics in these two pathogens, when compared to other European countries, is still very low.

      In March 1999 an unexpected high number of patients with atypical pneumonia was hospitalised in Hoorn. The diagnosis legionellosis was confirmed by the urine antigen test. An exploratory case control study suggested that the source should be found at the Westfriesian Flora (WF, a yearly exhibition organised from February 19 to 28 at Bovenkarspel with about 80,000 visitors). Subsequently, a national alert was sent out through the media, a case register was established and the outbreak investigation was started. This report describes the outbreak using the data in the case-register, as well as the results of the case-control study. Separate reports are published on the environmental investigation and the cohort study.The case-register includes 318 persons among whom 133 confirmed cases and 55 probable cases. The median age was 66 years (range 20-91 years), the male:female ratio 1.4. The day of onset was between February 25 and March16; the longest incubation period was 19 days, considerably longer than reported in literature. Except for one patient with COPD and recurrent pneumonia, all cases -regardless of earlier visits- visited the Flora on February 23 or later. The attack-rate per 100,000 visitors according to day of visit increased from 1.1 (February 21) to 53.4 (February 27). The highest number of hospital admissions was on March 12, the day of the national alert. The hospitalisation frequency was 86.7%; 19.1% of the patients required artificial respiration. Twenty-nine persons who were included in the case-register and had visited the Flora died, among whom 17 confirmed and four probable cases. The case-fatality rate for confirmed and probable cases amounted to 0,11.The findings strongly indicate that one or more sources of infection were contaminated with Legionella spp on or before February 21; subsequently multiplication of the agent in these sources led to increasing concentrations of Legionella spp in the exhibition halls. Three variables were included in the final models of all three measurements (questionnaire, sketches and floorplan) in the case-control study: the number of hours visiting the WF (OR 1.6, 2.2 and 1.8 respectively), smoking (OR 3.8, 6.0 and 10.9 respectively) and the number of hours visiting the consumer products fair with equal duration of the total visit (OR 0.7, 0.5 and 0.5 respectively). Age was included in the final model of questionnaire data (OR 1.1) and of floorplan data (OR 1.2). Halting at the whirlpool in hall 3 was an explanatory variable in the model of the sketch data (OR 2.6) as well as the floorplan data (OR 5.8). The variable 'whirlpool' was also associated with legionellosis in the analysis of questionnaire data. However, no distinction was made between the two whirlpools and the two bubblemats.Both from these results and from the results of the environmental investigation and the cohort study, it was concluded with reasonable certainty that the whirlpool in hall 3 was the source of the outbreak.
