      The trend of increasing Sexually Transmitted Infections (STI) has partly stabilised in 2005 in the STI sentinel surveillance network. Among men having sex with men (MSM), the number of STI diagnoses remained high and the HIV positivity rate has increased. Moreover, in the national HIV registry (HIV Monitoring Foundation), the number of HIV diagnoses was again highest among MSM. The number of HIV diagnoses among Dutch heterosexuals also slightly increased the last few years. Permanent alertness and innovative prevention and intervention methods are required to prevent further spread of STI and HIV. As of June 2006, a total of 11866 HIV cases were reported in the Netherlands; 970 diagnoses in 2005. MSM still accounted for the majority of the cases (52%, n=501). The number of HIV diagnoses among heterosexual of non-Dutch origin declined from 341 diagnoses in 2002 to 258 in 2005. The number of diagnoses among Dutch heterosexuals slightly increased from 86 in 2002 to 116 in 2005. In the STI sentinel surveillance network, the number of chlamydia and HIV cases increased by 15% and 25%, however the number of consultations increased as well. Gonorrhoea and syphilis slightly declined with 2% and 7%, respectively. Fifteen percent of all chlamydia, gonorrhoea, and syphilis cases among MSM were seen in HIV positives. Furthermore, in 2005 the percentage of ciprofloxacin resistance in gonococci has further increased up to 26%. In 2005, the evolution of the LGV outbreak in the Netherlands appears to have slowed down.

      Het aantal hivbesmettingen onder Antillianen, Surinamers en Kaapverdianen laat zien dat hiv een blijvend probleem is binnen deze gemeenschappen. De nieuwe cijfers van een gezamenlijk onderzoek van het RIVM en de GGD Rotterdam Rijnmond tonen dit aan. Bij Surinamers komt hivbesmetting bij 0,8 % van de mensen voor, bij Kaapverdianen 0,7%. Deze cijfers zijn gelijk aan die uit eerder onderzoek in 2002-2003. Bij de Antillianen is het 0,8 tot 3,2%.

      There is a potential for HIV and STI transmission from high risk groups into the general population in the Netherlands. This is one of the main conclusions of the HIV survey conducted among high risk populations in Amsterdam. HIV prevalence is 7% among commercial sex workers (CSW), 1% among Surinamese and 1% among Ghanese immigrants and 0% among Antillean immigrants and among prostitutes' clients. Sexual risk behaviour is high in all of these groups. The objectives of the survey were to assess the HIV prevalence and related risk behaviours among CSW, their clients and migrants from HIV endemic countries (HIV prevalence estimates > 1% in general population), and the potential of HIV transmission into the general population. This survey, designed as a pilot study, is part of the national HIV-surveillance. COMMERCIAL SEX WORKERS AND THEIR CLIENTS: HIV is most prevalent among transgender CSW (17%) and drug using CSW (11%). The HIV-prevalence among heterosexual CSW is 3%. Condom use with clients is high (79%), although condom failure is reported by one-third of the CSW. Prostitutes' clients regularly use condoms with CSW (82%); however, condom use with steady and casual partners is relatively low. None of the clients were HIV-positive. At least 10% of the clients used inconsistently condoms with CSW, steady and casual partners. This, in combination with the substantial HIV prevalence among CSW in Amsterdam (7%) suggests that transmission of HIV to the general population is possible. MIGRANTS FROM HIV-ENDEMIC COUNTRIES: Considerable sexual risk behaviour is reported in the migrant groups, more by men than by women: many sexual partners, concurrent partnerships and a low rate of condom use with steady and casual partners. Nine to sixteen percent of the migrants reported unsafe sex contacts with at least two partners in the previous six months. Due to sexual contacts within and between ethnic groups, further transmission of HIV and STI in the general population is a potential risk.

      Verspreiding van HIV en SOA kan mogelijk optreden doordat hoog-risicogroepen een brug vormen naar de rest van de bevolking in Nederland. Dit blijkt uit de HIV-survey die is uitgevoerd in Den Haag onder prostituees werkzaam in de raamprostitutie, clubs en op de tippelzone en onder migranten afkomstig uit HIV-endemische gebieden. HIV komt voor bij 3,5% van de prostituees. De HIV-prevalentie is 0,6% onder Antillianen, 0,7% onder Surinamers en 1,8% onder Ghanezen. Het doel van de survey was inzicht te verkrijgen in het voorkomen van HIV, seksueel risicogedrag en de potentie tot verspreiding hiervan bij prostituees en bij migranten afkomstig uit gebieden waar de HIV-prevalentie onder de algemene bevolking hoger dan 1% wordt geschat. De surveys maken onderdeel uit van de HIV-surveillance in Nederland. Prostituees: Bij de prostituees zijn HIV-infecties gevonden bij verslaafde vrouwen (22,2%) en bij transgenders (20%); bij de niet-verslaafde vrouwelijke prostituees zijn geen HIV-infecties aangetoond. Prostituees gebruiken vaak condooms met klanten (79%), maar ruim een derde rapporteert dat condooms regelmatig stuk gaan. Seksueel risicogedrag is hoger bij verslaafde prostituees en transgenders dan bij niet-verslaafde vrouwen. Gezien de hoge HIV-prevalentie onder transgenders en verslaafde vrouwen en het risicogedrag is de kans aanwezig voor de verspreiding van HIV vanuit deze groepen naar de rest van de bevolking. Migranten afkomstig uit HIV-endemische gebieden: Seksueel risicogedrag wordt vaak gerapporteerd en is hoger bij mannen dan bij vrouwen; ze rapporteren ongeveer twee keer zoveel partners, vaak meerdere partners tegelijkertijd, vaker seksuele contacten met losse partners in het land van herkomst en ze mixen vaker met partners van een andere etniciteit. Vrouwen gebruiken daarentegen minder vaak condooms, zowel bij vaste als bij losse partners. Van de migranten heeft 4-20% onbeschermde sekscontacten gehad met tenminste twee partners in de voorgaande zes maanden. Het risico van verdere verspreiding van HIV naar de rest van de bevolking in Nederland lijkt gering, echter door vele seksuele contacten onderling en tussen de verschillende etnische groepen is er een potentieel risico op verdere verspreiding van HIV/SOA.

This is one of the main conclusions of the HIV survey conducted among high risk populations in The Hague. HIV prevalence is 3.5% among commercial sex workers (CSW). Among immigrants, the HIV prevalence is 0.6% among Antillean immigrants, 0.7% among Surinamese and 1.8% among Ghanese immigrants. Sexual risk behaviour is substantial in all of these groups. The study was carried out to assess the HIV prevalence, related risk behaviours among CSW and among immigrants from HIV endemic countries (HIV prevalence is estimated > 1% in general population), and the potential of HIV transmission into the general population. This survey is part of the national HIV-surveillance. Commercial sex workers: Among CSW HIV is most prevalent among drug addicted CSW (22.2%) and among transgender CSW (20%). No HIV infections were found among other non-injecting female CSW. Condom use with clients is high (79%), although condom failure is reported by one third of the CSW. Sexual risk behaviour is more common among transgender and drug addicted CSW than among heterosexual CSW. This, in combination with the substantial HIV prevalence among transgender and drug addicted CSW in The Hague suggests that transmission of HIV to the general population is possible. Migrants from HIV-endemic countries: Considerable sexual risk behaviour is reported in the migrant groups, more by men than by women: many sexual partners, concurrent partnerships and sexual contacts in country of origin. However, women use less frequently condoms with both steady and casual partners. 4-20% of the migrants reported unsafe sex contacts with at least two partners in the previous six months.       Er bestaat een potentieel risico op verspreiding van HIV en SOA vanuit hoog-risicogroepen naar de rest van de bevolking in Nederland. Dit blijkt uit de eerste HIV-survey die is uitgevoerd in Rotterdam. Hierbij is gevonden dat de HIV-prevalentie onder prostituees 7% is, onder injecterende druggebruikers 10%, onder Kaapverdianen 1%, en onder Surinamers en Antillianen 0%. Het seksueel risicogedrag bij deze groepen is hoog. Doel van de survey was inzicht te verkrijgen in het voorkomen van HIV, seksueel risicogedrag en de potentie tot verspreiding hiervan bij prostituees, druggebruikers en migranten afkomstig uit HIV-endemische gebieden. De surveys zijn een onderdeel van de HIV-surveillance in Nederland.De HIV-prevalentie onder prostituees op de tippelzone (12%) is hoger dan bij prostituees in clubs (2%). Prostituees gebruiken vaak condooms met klanten (88%), echter deze condooms gaan regelmatig stuk (49%). Het aantal jaren werk in de prostitutie, transgenders en het spuiten van drugs waren geassocieerd met een HIV-infectie.Het seksueel risicogedrag onder migranten is hoog en hoger bij mannen dan bij vrouwen: dat wil zeggen veel partners, meer gelijktijdige partners en weinig condoomgebruik met vaste en losse partners. Door de vele seksuele contacten onderling en tussen de verschillende etnische groepen bestaat de kans op snelle verspreiding van HIV binnen deze groep migranten en naar de rest van de bevolking. De HIV-prevalentie onder injecterende druggebruikers is ongeveer hetzelfde gebleven ten opzichte van eerder onderzoek in 1997. Het spuitgedrag is minder risicovol geworden, echter het seksuele risicogedrag is hoog gebleven.De resultaten van de HIV-surveys worden gebruikt voor het preventiebeleid in Rotterdam

These are the main conclusions of the first HIV survey among high risk groups in Rotterdam. Among commercial sex workers (CSW) the HIV prevalence is 7%, 10% among injecting drug users (IDU), 1% among Cape Verde migrants and among Surinamese and Antillian migrants 0%. Sexual risk behaviour is high in these groups. The objectives were to assess the HIV prevalence, related risk behaviours among CSW , IDU and migrants from HIV endemic countries, and the potential of HIV transmission into the general population. This survey is part of the national HIV-surveillance.HIV prevalence among CSW in Rotterdam is higher in street-based prostitution (12%) than in establishment-based prostitution (2%). HIV prevalence among transgender sex workers is 20%. Condom use with customers is high (88%), although condom failure is reported regularly (49%). Condom use with steady and casual partners is low (15% and 25%, respectively). Therefore there is a potential for further spread of HIV in the general population. Number of years in prostitution, transgenders and injecting of drugs were associated with HIV. Substantial sexual risk behaviour is reported in the migrant groups, more in males than females: many sexual partners, concurrent partnerships and less frequent condom use with steady and casual partners (10% and 44%, respectively). Sexual contacts during visits to their countries of origin were reported by 19-32% of the respondents. Due to unsafe sex practices, concurrent partnerships, multiple partners and mixing between ethnic groups, further transmission of HIV and STI in the general population is a potential risk. HIV prevalence among IDU did not change significantly in comparison with prior studies in 1997. IDU shared less needles and syringes over time; however inconsistent condom use remained high: 85% with steady partners, 43% with casual partners. The potential for further spread is considerable due to the high HIV prevalence, the reported unsafe sex behaviour and the sexual mixing between IDU and non-IDU. Results of this survey will be used to target future prevention activities of the Municipal Health Service in Rotterdam. Repeated HIV-surveys are needed to monitor trends in sexual behaviour in high risk groups.
    • Registratie van soa en HIV consulten bij GGD-en en soa-poliklinieken: Jaarverslag 2002

      Bij de GGD-en en soa-poliklinieken wordt ten behoeve van de non-curatieve soa-bestrijding een registratie bijgehouden van de bezoekers met een soa-hulpvraag of HIV-testverzoek. In 2002 registreerden 37 GGD-en en twee soa-poliklinieken in totaal 23.353 consulten (stijging van 16% t.o.v. 2001), waarvan 49% bij mannen en 50% bij vrouwen. Van deze consulten werd bij 48% een soa-onderzoek uitgevoerd, bij 11% een HIV-test en bij 41% beide. In vergelijking met 2001 is het aantal testen voor soa alleen met 1% toegenomen, voor HIV alleen met 6% afgenomen en voor zowel soa als HIV met 53% toegenomen. Driekwart van de bezoekers was afkomstig uit Nederland. Het aantal soa steeg in 2002 met 11% tot 6.197. Bij 29% van de bezoekers werd een soa geconstateerd. De meest voorkomende soa was chlamydia; bij mannen gevolgd door genitale wratten, gonorroe en niet specifieke urethritis en bij vrouwen gevolgd door bacteriele vaginose, candidiasis en genitale wratten. Chlamydia steeg met 17%; bij mannen 19% en bij vrouwen 15%. Gonorroe steeg met 22%; bij mannen 18% en bij vrouwen 34%. Opvallend is het aandeel jonge vrouwen: 65% was jonger dan 25 jaar. Het aantal gevallen van syfilis steeg met 78%, vooral door een toename bij mannen van 122%; bij homo-en biseksuele mannen nam het aantal met 182% toe. Van alle vrouwen met syfilis was 46% werkzaam in de prostitutie. Het aantal HIV-testverzoeken steeg met 35% tot 12.151 waarbij 1,1% positief bleek te zijn. Bij de homo- en biseksuele mannen steeg het aantal HIV-testen met 47% en bij heteroseksuele mannen 28%. Het aantal HIV-positieven is met 111% meer dan verdubbeld. Van alle mannen met HIV was 76% homo- of biseksueel en van alle vrouwen was 63% afkomstig uit het buitenland waaronder 26% afkomstig uit sub-Sahara Afrika.

      Public health nurses at municipal health services (MHS) keep a register of patients attending the MHS or clinic for Sexually Transmitted Infections (STI) for diagnosis and treatment of STI or request for HIV-testing. Data are collected at a national level and are only registered for an actual STI or HIV consultation with confirmed diagnosis. Thirty-seven Health services and two STI clinics registered a total of 23,353 consultations with a confirmed diagnosis in 2002 (16% increase compared with 2001; men 19%, women 13%). The number of cases of diagnosed STI increased by 11% in 2002 (n= 6,197) compared with 2001; for men 18% and for women 6%. STI were diagnosed in 29% of the visitors who were tested for STI. Patients with a confirmed STI are more often men having sex with men (MSM). Chlamydial infection was the most common STI for both men and women. The next most common STI for men were genital warts, gonorrhoea and non specific urethritis; for women bacterial vaginitis, candidiasis and genital warts. The number of chlamydial infections increased by 17%; 19% for men and 15% for women. The number of gonorrhoea cases increased by 22%; 18% for men and 34% for women. The number of syphilis cases increased by 78%, mainly due to a large increase among men (122%). The remarkable rise of syphilis among MSM (182%) contributed highly to the overall increase. In total 12,151 patients requested HIV-testing (increase of 35%) of which 1,1% were positive. MSM were more often represented among the HIV-positives. About a quarter of the female HIV-positives originated from sub-sahara Africa.
