    • Het 131-I-gehalte van het rioolwater in Utrecht

      N.a.v. een soortgelijk onderzoek in West-Berlijn [1] werden in de periode maart-september 1979 metingen uitgevoerd naar het 131-I-gehalte van het effluent van de rioolwaterzuiveringsinstallatie te Utrecht. De gemiddelde concentratie is gering en bedroeg ca. 1 pCi/l bij een gemiddelde afvoer van 2 mCi/maand. Het 131-I-gehalte is vrijwel uitsluitend een gevolg van medische toepassing in het Academisch Ziekenhuis (AZU). Het gebruik van 131-I per hoofd van de bevolking bedraagt in Utrecht 0,2 muCi/jaar voor diagnostiek en 2 muCi/jaar voor therapie. De gemiddelde lozingsbijdrage a.g.v. diagnostiek kan worden geschat op 25% van de aangeschafte hoeveelheid en bedraagt voor Utrecht ca. 0,06 mu Ci/jaar. De gemiddelde lozingsbijdrage als gevolg van therapie wordt geschat op ca. 2% van de aanschaf ofwel 0,04 muCi/jaar. Vergeleken met schattingen voor West-Berlijn steken de hier gevonden resultaten gunstig af, hetgeen toegeschreven kan worden aan een lager hoofdelijk 131-I-verbruik, alsmede de lage procentuele lozingsbijdrage.
    • 16S rRNA-Based Microbiota Profiling Assists Conventional Culture Analysis of Airway Samples from Pediatric Cystic Fibrosis Patients.

      16S-based sequencing provides broader information on the respiratory microbial community than conventional culturing. However, it (often) lacks species- and strain-level information. To overcome this issue, we used 16S rRNA-based sequencing results from 246 nasopharyngeal samples obtained from 20 infants with cystic fibrosis (CF) and 43 healthy infants, which were all 0 to 6 months old, and compared them to both standard (blind) diagnostic culturing and a 16S-sequencing-informed "targeted" reculturing approach. Using routine culturing, we almost uniquely detected Moraxella catarrhalis, Staphylococcus aureus, and Haemophilus influenzae (42%, 38%, and 33% of samples, respectively). Using the targeted reculturing approach, we were able to reculture 47% of the top-5 operational taxonomical units (OTUs) in the sequencing profiles. In total, we identified 60 species from 30 genera with a median of 3 species per sample (range, 1 to 8). We also identified up to 10 species per identified genus. The success of reculturing the top-5 genera present from the sequencing profile depended on the genus. In the case of Corynebacterium being in the top 5, we recultured them in 79% of samples, whereas for Staphylococcus, this value was only 25%. The success of reculturing was also correlated with the relative abundance of those genera in the corresponding sequencing profile. In conclusion, revisiting samples using 16S-based sequencing profiles to guide a targeted culturing approach led to the detection of more potential pathogens per sample than conventional culturing and may therefore be useful in the identification and, consequently, treatment of bacteria considered relevant for the deterioration or exacerbation of disease in patients like those with CF. IMPORTANCE Early and effective treatment of pulmonary infections in cystic fibrosis is vital to prevent chronic lung damage. Although microbial diagnostics and treatment decisions are still based on conventional culture methods, research is gradually focusing more on microbiome and metagenomic-based approaches. This study compared the results of both methods and proposed a way to combine the best of both worlds. Many species can relatively easily be recultured based on the 16S-based sequencing profile, and it provides more in-depth information about the microbial composition of a sample than that obtained through routine (blind) diagnostic culturing. Still, well-known pathogens can be missed by both routine diagnostic culture methods as well as by targeted reculture methods, sometimes even when they are highly abundant, which may be a consequence of either sample storage conditions or antibiotic treatment at the time of sampling.

Hieruit blijkt dat Salmonella al een aantal jaren minder gezondheidsproblemen veroorzaakt, maar nog steeds, ná de Campylobacter-bacterie, de belangrijkste veroorzaker is van zoönotische ziekten in Europa. Databanken voor opslag van moleculaire typeringsdata Andere verslagen geven informatie over databanken die momenteel worden gebouwd door de EFSA en het European Centre for Disease Prevention and Control (ECDC). De EFSA-databank gaat informatie bevatten over moleculaire typering van ziekmakende bacteriën (pathogenen) die worden gevonden in voedsel, diervoeder en dieren. Die van het ECDC zal deze informatie bevatten van pathogenen gevonden bij de mens. Iedere bacteriestam heeft een eigen unieke moleculaire typering. Door de informatie uit de twee databanken te koppelen, kunnen bacteriestammen in producten en mensen worden achterhaald. Die kennis kan eraan bijdragen de bron te vinden van een, nationale of Europese, voedsel-gerelateerde uitbraak. De organisatie van de workshop is in handen van het EURL voor Salmonella, dat onderdeel is van het RIVM. De hoofdtaak van het EURL-Salmonella is toezien op de kwaliteit van de nationale referentielaboratoria voor deze bacterie in Europa.

      In deze studie is het urinaire 19-nortestosteron (NT) gehalte van met NT behandelde dieren vergeleken met de hoeveelheid resterend NT in de toedieningsplaats. Er zijn in totaal 136 dieren onderzocht, hoofdzakelijk kalveren, verdeeld over drie groepen. Geconcludeerd wordt dat er geen correlatie te vinden is tussen het urinaire NT-gehalte en de hoeveelheid NT restant in de toedieningsplaats. Tevens werd aangetoond dat indien de monsters urine van deze dieren in een keuringsprogramma onderzocht zouden zijn via een combinatie van hoge druk vloeistofchromatografie en radioimmunochemisch onderzoek (HPLC- RIA), er afhankelijk van de keuze van de beslissingsgrens voor voortgezet bevestigingsonderzoek een zeer groot aantal (70-90%) fout- negatieve resultaten zouden zijn verkregen.<br>

Deze systemen opereren voornamelijk binnen RIVM, alhoewel de steeds verdergaande samenwerking met externe instituten in binnen- en buitenland een bezinning over het informatie-beleid rechtvaardigt. Ten einde meer bekendheid te geven aan een breed publiek over kenmerken van basisbestanden die nodig zijn voor internationale, integrale milieustudies en -rapportages, wordt jaarlijks een catalogus uitgegeven.       We conducted a multicentre test-negative case-control study in 27 hospitals of 11 European countries to measure 2015/16 influenza vaccine effectiveness (IVE) against hospitalised influenza A(H1N1)pdm09 and B among people aged ≥ 65 years. Patients swabbed within 7 days after onset of symptoms compatible with severe acute respiratory infection were included. Information on demographics, vaccination and underlying conditions was collected. Using logistic regression, we measured IVE adjusted for potential confounders. We included 355 influenza A(H1N1)pdm09 cases, 110 influenza B cases, and 1,274 controls. Adjusted IVE against influenza A(H1N1)pdm09 was 42% (95% confidence interval (CI): 22 to 57). It was 59% (95% CI: 23 to 78), 48% (95% CI: 5 to 71), 43% (95% CI: 8 to 65) and 39% (95% CI: 7 to 60) in patients with diabetes mellitus, cancer, lung and heart disease, respectively. Adjusted IVE against influenza B was 52% (95% CI: 24 to 70). It was 62% (95% CI: 5 to 85), 60% (95% CI: 18 to 80) and 36% (95% CI: -23 to 67) in patients with diabetes mellitus, lung and heart disease, respectively. 2015/16 IVE estimates against hospitalised influenza in elderly people was moderate against influenza A(H1N1)pdm09 and B, including among those with diabetes mellitus, cancer, lung or heart diseases.

    • The 20th EU Interlaboratory comparison study in primary production (2017) : Detection of Salmonella in chicken faeces

      This report contains an erratum d.d. 17-7-2018 after page 34. In March 2017, the twentieth EURL-Salmonella interlaboratory comparison study on the detection of Salmonella in samples taken from the primary production stage was organised. Participation was obligatory for all EU Member State National Reference Laboratories (NRLs) responsible for the detection of Salmonella in these types of samples. Results. All laboratories were able to detect Salmonella in all the contaminated chicken faeces samples. Both the blank control sample and the positive control sample were analysed correctly by all laboratories. One laboratory made a labelling mistake and switched the process control for the positive control and therefore scored a 'moderate performance'. Blank samples containing chicken faeces not contaminated with Salmonella were correctly analysed as negative by almost all laboratories. One laboratory found Salmonella present in 3 of the 6 blank samples; this was indicated as a 'poor performance'. Participants. In total, 36 NRLs participated in this study: 29 NRLs from 28 EU Member States, 6 NRLs from other countries in Europe (EU candidate or potential EU candidate Member States and countries of the European Free Trade Association (EFTA)) and, on request of the European Commission, one NRL from a non-European country. EURL-Salmonella is situated at the Dutch National Institute for Public Health and the Environment (RIVM). The main task of EURL-Salmonella is to monitor and improve the performance of the national reference laboratories in Europe. Design of study. Each laboratory received a package of chicken faeces samples. The samples were contaminated by the EURL-Salmonella at two different Salmonella concentrations: high and low. The package also included blank samples without Salmonella. The laboratories were asked to analyse the samples according to the internationally prescribed method for the detection of Salmonella.
    • 20th EURL-Salmonella interlaboratory comparison study (2015) on typing of Salmonella spp

      The National Reference Laboratories (NRLs) of all 28 European Union (EU) Member States performed well in the 2015 quality control test on Salmonella typing. One laboratory was found to require a follow-up study after the initial test. Overall, the EU-NRLs were able to assign the correct name to 97% of the strains tested. In addition to the standard method for typing Salmonella (serotyping), sixteen laboratories performed typing at DNA level, using Pulsed Field Gel Electrophoresis (PFGE). This more detailed typing method is sometimes needed to trace the source of a contamination. For quality control, the participants received another ten strains of Salmonella to be tested by this method. Fourteen of the sixteen participating laboratories were suitably equipped to use the PFGE method. Since 1992, the NRLs of the EU Member States are obliged to participate in annual quality control tests which consist of interlaboratory comparison studies on Salmonella. Each Member State designates a specific laboratory within their national boundaries to be responsible for the detection and identification of Salmonella strains in animals and/or food products. These laboratories are referred to as the National Reference Laboratories (NRLs). The performance of these NRLs in Salmonella typing is assessed annually by testing their ability to identify twenty Salmonella strains. NRLs from countries outside the European Union occasionally participate in these tests on a voluntary basis. The EU-candidate-countries Former Yugoslav Republic of Macedonia and Turkey, and EFTA countries Iceland, Norway and Switzerland took part in the 2015 assessment. The annual interlaboratory comparison study on Salmonella typing is organised by the European Union Reference Laboratory for Salmonella (EURL-Salmonella). The EURL-Salmonella is located at the National Institute for Public Health and the Environment (RIVM), Bilthoven, the Netherlands.
