In opdracht van het Nederlands Astma Fonds werd onderzocht of verschillen in de prevalentie van astma en COPD tussen laag- en hoogopgeleiden kunnen worden verklaard door verschillen in leefstijl. Gegevens verzameld in de MORGEN-studie (uitgevoerd door het RIVM in 1993-1997) bij 19,555 Nederlandse volwassenen (20-59 jaar) werden geanalyseerd. Bij laagopgeleiden (lager onderwijs/lbo/(m)ulo/ mavo) kwamen astmasymptomen 1,4 maal en COPD-symptomen en luchtwegobstructie +- 1,9 maal zo vaak voor als bij hoogopgeleiden (hbo/wo). Na correctie voor actief en passief roken was de ratio van de prevalentie van astmasymptomen in laag- t.o.v. hoogopgeleiden 1,2. Voor kenmerken van COPD was de ratio gereduceerd tot +- 1,5. Na correctie voor roken, voedingsfactoren, en quetelet index (maat voor overgewicht) kwamen astmasymptomen, COPD-symptomen en obstructie nog respectievelijk 1,1, 1,2 en 1,3 maal zo vaak voor in laag- t.o.v. hoogopgeleiden. Sociaal-economische verschillen in het voorkomen van astma en COPD lijken dus voor een belangrijk deel verklaard te kunnen worden door verschillen in leefstijl.<br>To study whether socio-economic differences in chronic lung disease can be explained by lifestyle factors, data collected between 1993 and 1997 in the MORGEN-study (the monitoring project on risk factors and health in the Netherlands) in 19,555 adults (20-59 yr.) were analysed. The prevalence of asthma symptoms was 1.4 times and the prevalence of COPD symptoms and airway obstruction +-1.9 times higher in subjects with a low (intermediate secondary education or less) compared to a high (higher vocational or university education) educational level. After adjustment for active and passive smoking, the ratio of the prevalence of asthma symptoms in subjects with a low compared to a high educational level was 1.2. With regard to COPD symptoms and obstruction adjustment for smoking reduced the ratio to +-1.5. After adjustment for smoking, dietary factors and body mass index (indicator of overweight), asthma symptoms, COPD symptoms and obstruction were respectively 1.1, 1.2 en 1.3 times more prevalent in subjects with a low compared to a high educational level. Concluding, socio-economic differences in chronic lung disease may to a large extent be explained by differences in lifestyle.<br>
