In the period of March 1991 to December 1996, a total of 14,365 sera from pregnant women were investigated for estimating the risk of fetal Down syndrome and neural tube defects. Assays of alphafetoprotein (AFP), unconjugated estriol (uE3) and intact human chorionic gonadotropin (hCG) were performed with the radioimmunoassay kits, AmerlexM. Risk of Down syndrome and neural tube defects was calculated with the computer program , alpha. Retrospectively, the risk for trisomy 18 was also estimated on the basis of the same biochemical parameters. During the survey period, a new law on population research came into effect in 1996, making it necessary to license the offer of serum screening. As a consequence, those requesting risk estimations have shifted from obstetricians to midwives. In addition , a shift in the geographical origin of the samples from the Utrecht region to the western and southern parts of the Netherlands has occurred. As far as we currently know, 29 out of the 35 known Down syndrome cases were detected. Besides the Down syndrome other (numerical) chromosomal anomalies were detected. Since the maternal age of the pregnant women was very high and not representative of the normal Dutch population, a very high detection rate of Down's syndrome cases was found (86.3%); however, the false positive rate was also high (11.3%). It is possible to correct for the age distribution, and to calculate an expected false positive rate of 7.9% for the actual age distribution of the Dutch pregnant women, with a concommittant lower detection rate. All cases of neural tube defects have been detected, with the exception of one case of open spina bifida, which was probably caused by the use of valproate during pregnancy.Van 1991 t/m 1996 werden 14365 sera van zwangeren ingestuurd ten behoeve van de risicobepaling op Down syndroom en neuraalbuisdefecten via AFP, uE3 en hCG, de 'triple test'. Sinds invoering van de wet bevolkingsonderzoek (WBO) is het inzendpatroon sterk veranderd: er vond een decentralisatie plaats, meer inzenders zonden minder sera in. Dit heeft negatieve gevolgen voor kwaliteitscontrole en voor de voorlichting aan inzender en zwangere. De leeftijdsverdeling is anders dan bij de totale Nederlandse populatie zwangeren. Vooral zwangeren in de leeftijd van 34-37 jaar waren oververtegenwoordigd. Van de zwangeren had 1.5 % een verhoogd risico op NBD (> 2.50 MoM voor AFP), 11.3% een verhoogd risico voor Down syndroom, en 0.5% voor Trisomie 18. Zou men deze percentages omrekenen voor de leeftijdsverdeling van de toenmalige totale populatie Nederlandse zwangeren, dan worden deze respectievelijk 1.5%, 7.9% en waarschijnlijk 0.4%. Van de tot nu toe bekende Down Syndroom gevallen was in 29 van de 35 gevallen het risico op Down syndroom verhoogd. De verschillende chromosoomafwijkingen worden, afhankelijk van een eventuele associatie met andere (open) defecten, gevonden via een verhoogd risico op neuraalbuisdefecten, op Down Syndroom of op trisomie 18. De voorlopige resultaten zijn dus indrukwekkend, de sensitiviteit is 86.4% voor alle via de 3 risico-berekeningen gevonden afwijkingen, de specificiteit is 87.1%, de positief voorspellende waarde is 3.03%, en de negatief voorspellende waarde is 99.93%. Voor een definitieve conclusie met betrekking tot de statistische parameters is het wachten op meer post partum gegevens. Het binnenkrijgen en afwerken van de gegevens kan nog veel tijd in beslag nemen.
