dc.description.abstractLeefstijlbegeleiding op maat levert een aanzienlijke gezondheidswinst op bij mensen met diabetes of bij mensen die door verstoorde glucosetolerantie (IGT) en/of overgewicht een hoog risico lopen de ziekte te krijgen. De leefstijlbegeleiding bestaat uit persoonlijke adviezen over bewegen en voeding. Begeleiding tot 400 euro per persoon levert meer gezondheidswinst op dan het huidig basispakket en is kosteneffectief. Dit blijkt uit literatuuronderzoek en modelberekeningen van het RIVM. De resultaten zijn relevant voor de samenstelling van het basispakket van de zorgverzekering. Op dit moment bevat dit maximaal vier consulturen per jaar met een dietist op medische indicatie. In Nederland komen ruim een miljoen mensen tussen de 30 en 70 jaar in aanmerking voor leefstijlbegeleiding. Het gaat om mensen met overgewicht en een hoge bloeddruk, met een verstoorde glucosetolerantie (IGT) of met diabetes type 2. Mensen met overgewicht (en zeker obesitas) en/of met IGT lopen een groter risico om diabetes te ontwikkelen. Naar verwachting is ongeveer een derde van de groep bereid om een 'leefstijlkuur' te gaan volgen. Het RIVM berekende van vier leefstijlpakketten de kosteneffectiviteit in kosten per QALY. Een QALY staat gelijk aan een gewonnen levensjaar in goede gezondheid. De kosten van de pakketten varieerden van 150 tot 1150 euro per patient in het eerste behandeljaar. Voor mensen met overgewicht kost een QALY 900 tot 8.600 euro; voor patienten met diabetes type 2 is dat 3.900 tot 26.100 euro. Deze ramingen hangen af van de kosten van het pakket en de leeftijd van de patient en nemen de medische kosten in gewonnen levensjaren mee. Zonder deze kosten kan de leefstijlbegeleiding kostenbesparend zijn bij mensen met overgewicht of IGT. Als leefstijlbegeleiding daadwerkelijk in de praktijk geimplementeerd gaat worden dan moet de kwaliteit daarvan goed gegarandeerd worden. Er is goede inbedding noodzakelijk in de gebruikelijke zorg.
dc.description.abstractLifestyle counseling is accompanied by significant health gains for patients with type II diabetes mellitus (DM2) or persons with increased risk for DM-2 due to impaired glucose tolerance (IGT) and/or overweight. The counseling consists of personal advices concerning physical activity and a healthy diet. Expanding lifestyle counseling up until EUR 400 per person in the first year will result in more health gains compared to the current standard health ensurance package and this is costeffective. These results are based on literature research and model calculations by the National Institute for Public Health and the Environment (RIVM). In The Netherlands approximately over 1 million 30-to-70-year-old persons may be indicated for lifestyle counseling. These are persons with overweight and hypertension, IGT or DM-2. Persons having overweight (especially obesity) and/or IGT are at increased risk for DM-2. It is estimated that about one third of this group is motivated to start with a lifestyle counseling programme. Cost effectiveness was calculated for four packages of lifestyle counseling (varying EUR 150 to 1150 per patient in the first year) in costs per QALY gained. A QALY is a life year saved in good health. For people with overweight cost effectiveness was EUR 900-8600 per QALY and for people with diabetes mellitus type II EUR 3900-26100 per QALY, dependent on the package and the peoples' age and including the costs for health care in lifeyears saved. Without these costs lifestyle counseling may be cost saving for persons with overweight and/or IGT. RIVM stresses that actual implementation should be accompanied by quality assurances and that the lifestyle counseling should be well incorporated within usual health care procedures.
