dc.description.abstractOngezond gedrag leidt tot een verlaging van de (gezonde) levensverwachting. Wanneer we de risicofactoren roken, overgewicht en lichamelijke inactiviteit met elkaar vergelijken blijkt dat roken tot het grootste verlies van (gezonde) levensverwachting leidt. De hypothetische uitbanning van roken en andere risicofactoren leidt dus tot gezondheidswinst voor de bevolking. Deze uitbanning van ongezonde gedragingen leidt echter ook tot een stijging van de toekomstige zorgkosten. De hogere zorgkosten op lange termijn is de prijs die betaald moet worden voor het succes van preventie, namelijk een gezondere en langer levende bevolking. In dit rapport is met behulp van het RIVM Chronische Ziekten Model (CZM) de ziektelast van de risicofactoren roken, overgewicht en lichamelijke inactiviteit geschat. Deze drie risicofactoren vormen drie belangrijke pijlers van het preventiebeleid dat door het Ministerie van VWS gevoerd wordt. De ziektelast wordt onder andere uitgedrukt in verlies aan levensjaren als gevolg van ongezonde leefgewoonten en in verlies aan kwaliteit van het leven. Daarnaast wordt de doelmatigheid van een elftal interventies gericht op het bevorderen van een gezonde leefstijl op een rijtje gezet. Theoretische eliminatie van de risicofactoren roken, overgewicht en lichamelijke inactiviteit in de huidige bevolking leidt tot een stijging van de gezonde levensverwachting van respectievelijk 1,6, 0,8 en 0,4 jaar. Dit heeft echter tot gevolg dat de totale zorgkosten voor de huidige Nederlandse populatie over een periode van 100 jaar stijgen met respectievelijk 6,2%, 2,0% en 2,1%. Tevens zou eliminatie van de risicofactoren roken, overgewicht en lichamelijke inactiviteit in de huidige bevolking leiden tot een minder sterke stijging in het percentage mensen met diabetes. Veel interventies gericht op het stoppen met roken, afvallen en meer bewegen kunnen als doelmatig worden bestempeld. De kosten-effectiviteit van de maatregelen die we hebben doorgerekend met het CZM blijft bijna in alle gevallen onder de in Nederland vaak gehanteerde drempelwaarde van Euro 20.000,- per gewonnen gezond levensjaar. Dit geldt ook wanneer we rekening houden met de extra kosten van medische zorg in gewonnen levensjaren.

Comparison of the three risk factors smoking, obesity and physical inactivity reveals that smoking results in the biggest loss in (healthy) life expectancy. The hypothetical elimination of smoking, overweight and physical inactivity would result in health gains for the Dutch population. However, as a consequence of increases in life expectancy, health care costs will increase. This holds even if we account for the savings in health care costs of diseases related to the three risk factors. The hypothetical elimination of smoking has the largest effect on future health care costs. The increase in health care costs in the long run can be considered as the price society has to pay for the result of successful prevention, namely a healthier and longer living population. Using the RIVM Chronic Disease Model (CDM), this report presents estimates of the burden of disease that can be attributed to smoking, overweight and physical inactivity. These three risk factors are key elements in public health policy of the Dutch Ministry of Health, Welfare and Sport. In this report, the burden of disease is expressed in loss of (healthy) life years and also in loss of quality of life. Using the CDM we tried to answer the question what health gains could be achieved if the three risk factors could be eliminated. Furthermore, the influence of elimination of risk factors on future health care costs is estimated. Finally, an inventarisation is made of the cost effectiveness of interventions targeted at these risk factors. Theoretical elimination of the risk factors smoking, overweight and physical inactivity in the current population leads to an increase in healthy life expectancy of 1.6, 0.8 and 0.4 years, respectively. As a consequence, total health care costs for the current Dutch population over a period of 100 years will increase with 6.2%, 2.0% en 2.1%, respectively. Furthermore, elimination of smoking, overweight and physical inactivity would moderate the expected increase in the percentage of the population with diabetes. Many interventions targeted at smoking cessation, weight loss and more physical activity are cost-effective. Using the CDM, we have modelled the cost-effectiveness of 11 interventions targeted at improvements in life-style. Most of these interventions remain below the much quoted threshold of Euro 20,000.- per healthy life year gained, also if we accounted for the additional costs of medical care in life years gained following successful prevention. Most data on cost-effectiveness of interventions are available for interventions focusing on smoking cessation. This is related to the fact that those interventions have been evaluated relatively better with regard to their effectiveness. We conclude that even if health care costs in life years gained are taken into account a lot of interventions targeted at lifestyle changes can be termed cost-effective.
