dc.description.abstractOm na te gaan in hoeverre de autoclaven op de centrale sterilisatie afdelingen in de Nederlandse ziekenhuizen in staat zijn om de helixtest conform Europese norm EN867-5 tot een positief resultaat te brengen is een praktijkonderzoek uitgevoerd in 20 ziekenhuizen. In totaal zijn 476 testresultaten verkregen, waarvan slechts 59% positief was. Na analyse van de testresultaten blijkt de uitslag van de testhelixen bepaald te worden door het type testhelix, het type lading waarin de testhelix wordt toegevoegd en het type sterilisatieproces. Twee van de vier typen testhelixen geven minder vaak een positief testresultaat. Alle typen testhelixen komen minder vaak tot een positief resultaat indien ze worden opgenomen in een testlading. Een licht instrumentennet met een massa van 2 kg bleek in dit kader het meest kritisch te zijn. Het type sterilisatieproces waarin gebruikt wordt gemaakt van de 'klassieke' ontluchtingsfase, bestaande uit drie diepe vacuumpulsen afgewisseld met stoompulsen, blijkt een significant beter resultaat te geven dan andere type sterilisatieprocessen. De helixtest kan een waardevolle bijdrage leveren aan de optimalisatie van het sterilisatieproces en de stoomkwaliteit ten behoeve van de sterilisatie van holle instrumenten. Het rapport geeft informatie en aanbevelingen om tot implementatie van de helixtest te komen.
dc.description.abstractThe RIVM performed the helix test in the autoclaves of 20 hospitals. Only 59% of the test results were positive, while the Bowie and Dick test was positive in all cases. The test helix added to a light instrument tray proved to be a particular challenge to the sterilisation process. One of the measures for infection prevention in hospital is the sterilisation of the surgical instruments by steam sterilisation. For this sterilisation method it is important that steam is able to get to all surfaces of the instruments and to penetrate into hollow instruments. European standardisation committee CEN TC102 developed the helix test to demonstrate steam penetration into hollow devices. This method utilises a long narrow tube. Steam penetration throughout the length of the tube is demonstrated with an indicator at the end of the tube. This test method is essentially different from the currently used test method that utilises a textile pack; the Bowie and Dick test. Nowadays the vast majority of the hospitals do not sterilise textile packs, but there is a distinct increase in the number of hollow instruments that is reprocessed, eg. laparoscopic instruments. Hospitals should adopt their sterilisation indicators to the type of steriliser load and are advised to evaluate the performance of the sterilisers with the helix test added to a light instrument tray.
