De mens wordt in toenemende mate, via de voeding, blootgesteld aan nitraat. De toxiciteit van nitraat is laag. In het lichaam vindt echter omzetting plaats van nitraat in nitriet in het proximale deel van de tractus digestivus. Nitriet is toxischer dan nitraat. Bij evaluatie van de gezondheidsrisico's van nitraat moet dan ook rekening worden gehouden met de omzetting in nitriet en de orale biobeschikbaarheid en toxiciteit van nitriet in het lichaam. De planning was om een orale biobeschikbaarheidsstudie van nitriet uit te voeren. Nitriet wordt in het bloed echter snel omgezet in nitraat onder de vorming van methemoglobine. Vanwege de snelle eliminatie van nitriet zijn dan ook relatief hoge nitriet doseringen nodig om de biobeschikbaarheid te bepalen. Vanwege de door nitriet geinduceerde bloeddrukverlaging en methemoglobine vorming zijn er echter grenzen aan de hoeveelheid nitriet die nog veilig kan worden toegediend tijdens een biobeschikbaarheidsstudie. De geplande biobeschikbaarheidsstudie werd daarom voorafgegaan door een pilot-studie waarin aan 3 proefpersonen opklimmende, intraveneuze doseringen van natriumnitriet gegeven werden. Het doel was om de maximale dosis te vinden die nog veilig aan proefpersonen kan worden toegediend, te weten, een dosis die ongeveer 10-15% methemoglobine induceert. Geconcludeerd kan worden dat een dosis van 0.12 mmol NaNO2 per mmol Hemoglobine (tussen 290 en 370 mg natriumnitriet), waarbij 10.8% metHb werd geinduceerd, de hoogste, nog veilige, natriumnitriet dosis is voor proefpersonen.<br>There is widespread concern about exposure to nitrate from dietary sources and its potential risk to human health. The toxicity of nitrate is low. However, in the proximal part of the gastro-intestinal tract, part of the nitrate is converted to nitrite, a more toxic compound. In considering the health risks of ingested nitrate it is, therefore, important to take into account the formation, oral bioavailability and toxicity of nitrite. A healthy volunteer study was planned to investigate the oral bioavailability of nitrite from an aqueous solution. However, in blood nitrite is rapidly oxidized by hemoglobin to yield methemoglobin and nitrate. Calculating the oral bioavailability of nitrite from the plasma nitrite concentration is hampered and requires a substantial nitrite dose to be administered to healthy volunteers. Because of the vasodilating and methemoglobin inducing potential of nitrite there are limitations to the nitrite dose which can safely be administered to volunteers in a bioavailability study. The bioavailability study was preceded by a single , intravenous, ascending dose study to investigate the maximum nitrite dose which can safely be administered in vivo; i.e. inducing approximately 10 to 15% metHb. This study demonstrates that a sodium nitrite dose of 0.12 mmol NaNO2 per mmol Hb (between 290 and 370 mg) and inducing approximately 10.8% methemoglobin in the blood, is the maximum tolerated dose for adult volunteers.<br>
