dc.description.abstractKinderen die sinds 2005 met het nieuwe kinkhoestvaccin zijn gevaccineerd, maken meer antistoffen tegen de ziekte aan dan bij gebruik van het oude vaccin. Aangezien het effect van het vaccin pas op lange termijn merkbaar is in de bevolking, is nu nog niet duidelijk of hierdoor het aantal gevallen van kinkhoest zal dalen. Dit blijkt uit een onderzoek van het RIVM naar het effect van de wisselingen van het kinkhoestvaccin tussen 2004 en 2008 in het Rijksvaccinatieprogramma (RVP) bij ruim 400 kinderen van 1 jaar oud. Sinds 2005 wordt aan 1-jarigen een nieuw kinkhoestvaccin toegediend. Bovendien is in 2001 op een leeftijd van 4 jaar een herhaalenting toegevoegd aan het prikschema. Aanleiding voor de aanpassingen is een forse toename van het aantal kinkhoestgevallen sinds de jaren negentig. Het RIVM beveelt aan om niet alleen de samenstelling van het RVP in het eerste levensjaar te onderzoeken, maar ook het tijdschema waarop de vaccins worden toegediend. Momenteel geldt voor het eerste jaar het zogeheten 3+1-schema, waarbij kinderen op 2, 3, 4 maanden een prik krijgen, gevolgd door een 'booster' op de leeftijd van 11 maanden. De in 1999 ingevoerde vervroeging van 3, 4, 5 naar 2, 3, 4 maanden lijkt minder effectief te zijn. Uit het onderzoek blijkt ook dat een gelijktijdige toediening van combinatievaccins zijn beperkingen kent. Een te groot aantal kan de effectiviteit van de vaccins onverwacht negatief beinvloeden. Elke aanpassing in het RVP verdient daarom een degelijke evaluatie. Tegelijkertijd is het door de vele wisselingen van kinkhoestvaccins in de afgelopen tien jaar moeilijker geworden om de bescherming op lange termijn van een type vaccin te meten.
dc.description.abstractChildren who have been vaccinated with the new pertussis vaccine since 2005 produce more antibodies directed against whooping cough compared with the old vaccine. Because the effect of this improved vaccination response will only be noticable in the population at long term it is not clear at present whether or not the incidence of whooping cough will decrease. This is the main result of an RIVM study to monitor the effect of the pertussis vaccine changes in the national immunisation program (NIP) between 2004 and 2008 conducted in over 400 children of 1 year of age. Since 2005 a new pertussis vaccine has been administered to children in their first year of life. This followed the 2001 introduction of a booster vaccination to the NIP schedule at the age of 4 years. The reason for these changes was the sharp increase in the incidence of pertussis cases from 1996 onwards. The RIVM recommends investigating not only the composition of the NIP in the first year of life but also the used vaccination schedule. At present, the so-called 3+1 schedule is employed during the first year of life where children are vaccinated at 2, 3 and 4 months followed by a booster at 11 months. The change made to the schedule in 1999 - from 3, 4, 5 months to 2, 3, 4 months - appears to be less effective. The study has also revealed that the simultaneous administration of combination vaccines has limitations. If too many vaccines are combined this can unexpectedly have a negative influence on the effectivity. Consequently, all changes made to the NIP deserve a good serological evaluation. At the same time, the many changes of pertussis vaccines in the NIP during the last ten years have made it difficult to study the protection of one type of vaccine at long term.
