dc.description.abstractVWS-beleid gaat er vanuit dat mensen eerder doorgeleid kunnen worden naar (lichte) zorg als gezondheidsrisico's vroegtijdig worden opgespoord. Ziekte en daarvoor benodigde intensievere zorg kunnen dan worden uitgesteld of voorkomen. In dit verband heeft het RIVM in opdracht van het ministerie van VWS verkend welke elementen bijdragen aan het succes van lokale initiatieven om gezondheidsrisico's vroegtijdig op te sporen. Hiervoor zijn mensen uit het veld (onderzoek, beleid, praktijk) geïnterviewd en is op kleine schaal literatuuronderzoek gedaan. Gezien het verkennende karakter dienen de resultaten met enige voorzichtigheid geïnterpreteerd te worden. Een vaak genoemd element dat bij kan dragen aan het succes van initiatieven is integrale samenwerking. Daaronder wordt verstaan dat partijen, zowel binnen de zorg (zoals wijkverpleegkundigen en huisartsen) als daarbuiten (bijvoorbeeld buurtcoaches en medewerkers in het onderwijs), samenwerken bij het vroegtijdig opsporen van de doelgroep en de geboden zorg. Daarvoor is het onder andere van belang dat er voldoende draagvlak is bij alle betrokken partijen, net als één partij die de regie op zich neemt. Een ander bepalend element is dat het initiatief goed aansluit bij de doelgroep. Ook structurele financiering is genoemd als randvoorwaarde voor succes. In de verkenning stonden de volgende initiatieven centraal: het opsporen van gezondheidsrisico's in de wijk, van gezondheidsproblemen bij kwetsbare ouderen, van problematisch alcoholgebruik onder jongeren, van seksueel overdraagbare aandoeningen en ten slotte van (hoog risico op) hart- en vaatziekten en diabetes. Het onderzoek maakt ook duidelijk dat er onderzoek nodig is om te bewijzen dat lokale initiatieven om gezondheidsrisico's vroegtijdig op te sporen daadwerkelijk de gezondheid van mensen verbeteren. Het is daarom van belang om ingezette interventies te monitoren en te evalueren. Daarnaast is meer inzicht nodig in de behoeften die zorgverleners hebben om de initiatieven te kunnen uitvoeren, bijvoorbeeld ICT om gemakkelijk en snel informatie uit te wisselen.
dc.description.abstractOne of the points that Dutch healthcare policy (Ministry of Health, Welfare and Sport, VWS) embraces is that people can be referred to health care services earlier when health risks are recognized at an early stage. This can lead to the postponement or avoidance of illness and more intensive types of care having to be given. With this in mind and at the request of VWS, the National Institute for Public Health and the Environment (RIVM) has studied which elements contribute to the success of local initiatives aimed at the early detection of health risks. For this purpose, professionals in health care, including researchers, policy-makers and clinicians, were interviewed and a small scale literature study was done. Considering the exploratory nature of this study, the results should be interpreted with due caution. One point that was mentioned frequently with regard to the success of initiatives is mainstreamed collaboration. What is meant by this term is that the parties involved, both in health care (e.g. district nurses and general practitioners) and those not in health care (e.g. people working at local level and in education) work together for the early detection of target groups and for the care that will be needed. One of the important points to remember here is that there must be sufficient support for all the parties involved and that one party must take charge. Another determining factor is that the initiative links up with the target group. Furthermore, structural financing was mentioned as a factor of success. The survey highlighted the following initiatives: the detection of health risks at neighbourhood level, health problems in the vulnerable elderly, problems associated with alcohol in youths, sexually transmittable diseases and finally people at high risk of cardiovascular disease and diabetes. The study also showed that further research is needed to prove that local initiatives aimed at the early detection of health risks do actually improve people's health. Implemented interventions should be monitored and evaluated. In addition, more information is needed about what care providers need in order to implement the initiatives, for example, ICT measures to enable information exchange to be fast and easy.
