dc.description.abstractBij lokale preventieve gezondheidsinterventies werken de zorg en de gemeente soms samen. Dit gaat niet altijd vanzelf, onder andere omdat de geldstromen niet helder zijn en de kosten en baten bij verschillende partijen terechtkomen. Een voorbeeld van een lokale interventie is Welzijn op recept. Hierbij verwijst de huisarts mensen met lichte psychische klachten, zoals rouw of eenzaamheid, door naar sociale activiteiten in de buurt. Betrokken organisaties hebben de indruk dat deelnemers van Welzijn op recept zich beter voelen en minder vaak onnodig de huisarts bezoeken. Om die indruk te bevestigen moet worden onderzocht of de interventie effect heeft. Dit blijkt uit een verkenning van het RIVM naar het nut van een kosten-baten rekentool voor Welzijn op recept, in opdracht van het ministerie van VWS. Een rekentool die de kosten en baten in geld uitdrukt heeft voor betrokken organisaties weinig meerwaarde. Aanbieders van zorg en ondersteuning willen vooral de interventie beter organiseren en de effecten op de deelnemers vergroten. Succesvolle elementen van Welzijn op recept zijn de verwijzing van de huisarts, korte lijnen en goede samenwerking tussen de huisarts en de welzijnscoach. Dit organisatievraagstuk is belangrijker dan de financiering. Landelijk gezien kan een rekentool geldstromen inzichtelijk maken en structuur aanbrengen in de financiering van het grijze gebied tussen zorg en welzijn. Maar ook daarvoor is eerst meer kennis nodig over de effectiviteit van de interventie. VWS heeft ook gevraagd wat de beste maatregel is om overgewicht bij kinderen tegen te gaan. Hoewel verschillende interventies voeding, beweging en gedrag combineren, variëren ze nogal in opzet en effectiviteit. Zo zijn er interventies voor thuis en op school. Ook verschillen de resultaten. Om te kunnen zeggen welk type het meest effectief is, is uitgebreid literatuuronderzoek nodig.
dc.description.abstractHealthcare service providers and municipalities sometimes work together on local preventive health interventions. This does not always go as smoothly as desired, partly because funding lines are unclear and the costs and benefits end up with different parties. One example of a local intervention is 'Social prescribing'. A general practitioner may refer someone with mild psychological complaints, due to bereavement or social isolation for example, for participation in social activities in the community. The organisations involved believe that participants in 'Social prescribing' feel better and are less likely to visit their general practitioner unnecessarily. In order to confirm this, it is necessary to investigate whether this intervention is effective. This is the conclusion of a study by the RIVM focusing on the usefulness of a cost-benefit calculation tool for 'Social prescribing', commissioned by the Ministry of Health, Welfare and Sport. A calculation tool that expresses the costs and benefits in monetary terms would have little added value for the organisations involved. Healthcare providers mainly want to organise the intervention better and increase its effectiveness for participants. The successful elements of 'Social prescribing' are the referral of the general practitioner, short communication lines and good collaboration between the general practitioner and the welfare coach. These organisational issues are more important than funding. From a national point of view, a calculation tool could provide a better insight into funding flows and provide more structure in the financing of this grey area between healthcare and welfare. However, this also requires more knowledge about the effectiveness of the intervention. The Ministry of Health, Welfare and Sport has also asked what the best measure for reducing obesity in children would be. Although various interventions combine nutrition, exercise and behaviour, they vary in their design and effectiveness. For example, there are home-based and school-based interventions. The results also differ. In order to determine which type is most effective, a systematic literature review is required.
