Gebruiksvoorschrift Storybuilder-MHC : Wijze waarop kenmerken van incidenten met gevaarlijke stoffen bij majeure risicobedrijven worden ingevoerd in Storybuilder-MHC

Abstract

Sinds 2009 analyseert het RIVM oorzaken en omstandigheden van incidenten en ongevallen met gevaarlijke stoffen bij chemische bedrijven. Het RIVM verzamelt hierover een groot aantal kenmerken en analyseert die vervolgens met het model Storybuilder-MHC. Het RIVM heeft nu een gebruiksvoorschrift gemaakt dat beschrijft welke informatie voor de analyses wordt gebruikt. Ook staat erin hoe de kenmerken van de incidenten in het model worden vastgelegd. Het gebruiksvoorschrift zorgt ervoor dat incidenten op dezelfde manier worden geanalyseerd. De analyses zijn bedoeld om ontwikkelingen en patronen in incidenten te ontdekken. De inzichten helpen inspecteurs bij hun werk. Bedrijven kunnen inzichten uit de analyses gebruiken om ongevallen te voorkomen. De analyses worden in opdracht van het ministerie van Sociale Zaken en Werkgelegenheid (SZW) uitgevoerd. Storybuilder-MHC bevat onder andere kenmerken over het soort bedrijf (zoals raffinaderij), het type incident (bijvoorbeeld vrijgekomen gevaarlijke stoffen), de oorzaken van het incident en de kenmerken van het slachtoffer en zijn letsel (zoals de ernst ervan). De analisten halen deze informatie uit rapporten die zijn opgesteld door de Inspectie Sociale Zaken en Werkgelegenheid (Inspectie SZW) of de Onderzoeksraad voor Veiligheid (OvV). Zij leggen kenmerken per incident vast. Storybuilder-MHC is een onderdeel van het analysemodel Storybuilder. Hierin analyseert het RIVM oorzaken en omstandigheden van ernstige arbeidsongevallen bij bedrijven.

RIVM has been analysing the causes of, and circumstances surrounding, incidents and accidents involving hazardous substances at chemical companies since 2009. RIVM collects a large number of characteristics regarding these events and analyses them using the Storybuilder-MHC model. RIVM has now drawn up user instructions that describe the information used for the analyses. It also states how the characteristics of incidents are recorded in the model. These user instructions ensure that incidents are analysed in the same way. The purpose of the analyses is to discover developments and patterns in incidents. The insights resulting from the analyses help inspectors to carry out their work. Companies can use them to prevent accidents. The analyses are commissioned by the Ministry of Social Affairs and Employment (SZW). Among other things, Storybuilder-MHC contains characteristics of the type of company involved (such as a refinery), the type of incident (such as the release of hazardous substances), the cause of the incident and the main characteristics of victims and their injuries (such as severity). The analysts find this information in reports drawn up by the Social Affairs and Employment Inspectorate (SZW Inspectorate) or the Dutch Safety Board (OvV). The analysts record characteristics for each individual accident. Storybuilder-MHC is an element of the analytical model Storybuilder. RIVM uses it to analyse the causes of, and circumstances surrounding, serious occupational accidents at companies.
