dc.description.abstractThis document provides an overview of the occurrence of the target diseases included in the National Immunization Programme (NIP) of the Netherlands (period 1997-2002). Currently children are immunized against diphteria, pertussis, tetanus, poliomyelitis, invasive Haemophilus influenza type b, mumps, measles, rubella, invasive meningococcal infections type C and hepatitis B. Information from different surveillance systems was used to describe trends in mortality and morbidity for the period 1997-2002. The main sources used were the Central Bureau of Statistics for mortality figures, the National Medical Registration for hospital admissions and the Medical Inspectorate of health for notifications of the notifiable diseases. It is evident that the diseases included in the immunization programme are under control due to high immunization coverage levels; the European Region has been declared polio-free and very few cases of diphteria, tetanus, mumps and rubella were reported for the given period in the Netherlands. However, several findings stress the importance of maintaining high vaccination coverage levels and continuous monitoring of the target diseases. Pertussis, for example, shows that a disease can still be endemic with epidemic peaks despite high vaccination coverage levels. Furthermore, the recent outbreak of measles shows that clustering of unvaccinated individuals plays an important role in the (re)occurrence of disease. The essential dynamics of an immunization programme were illustrated by the actions taken in reponse to the considerable increase in cases of meningococcal infections. A nationwide vaccination campaign was organized and vaccination against meningococcal disease type C was succesfully introduced in the NIP in 2002. The report concludes with recommendations for the different target diseases.
dc.description.abstractHet Rijksvaccinatieprogramma (RVP) is een preventieprogramma dat sinds 1957 door de overheid wordt aangeboden. Het doel van het RVP is kinderen te beschermen tegen de volgende ernstige infectieziekten: difterie, kinkhoest, tetanus, poliomyelitis, infecties door haemophilus influenza type b, bof, mazelen, rodehond, meningokokken C en hepatitis B (voor bepaalde groepen zuigelingen). Het huidige rapport geeft een kort overzicht van het voorkomen van de infectieziekten waartegen in het RVP gevaccineerd wordt (periode 1997-2002). Ziekte- en sterftecijfers uit verschillende bronnen over de periode 1997 - 2002 worden kort beschreven en eventuele epidemische verheffingen of waargenomen trends worden toegelicht. Het RVP is een succesvol programma. Europa is sinds 2002 officieel vrij verklaard van endemische polio en voor de periode 1997-2002 werden in Nederland slechts enkele gevallen van difterie, tetanus, bof en rodehond gemeld. Aangezien de kans dat zich een epidemie voordoet van een van de infectieziekten waartegen binnen het RVP gevaccineerd wordt bepaald niet ondenkbaar is, zeker niet zolang deze ziekten wereldwijd nog niet zijn uitgeroeid, zijn het handhaven van een hoge vaccinatiegraad en een continue monitoring van het voorkomen van de doelziekten uit het RVP van essentieel belang. Kinkhoest laat zien dat ondanks een hoge vaccinatiegraad een ziekte nog steeds endemisch kan zijn met epidemische verheffingen. De mazelenepidemie van 1999/2000 bewijst wederom dat ondanks de gemiddeld hoge vaccinatiegraad clustering van ongevaccineerde individuen in Nederland een daadwerkelijk risisco vormt. Voor een blijvend succes van het RVP is continue evaluatie en eventuele aanpassing van het programma noodzakelijk. Recent voorbeeld daarvan is de introductie van de vaccinatie tegen meningokokken C naar aanleiding van de toename van meningokokken C in Nederland in 2001/2002. Conclusies voor de verschillende doelziekten uit het RVP staan vermeld voor alle ziektes apart en worden nogmaals samengevat in het laatste hoofdstuk.
