Sexually transmitted infections in the Netherlands in 2021

Abstract

In 2021 hebben meer mensen zich bij een Centrum voor Seksuele Gezondheid (CSG) laten testen op seksueel overdraagbare aandoeningen (soa) dan in 2020. Dit aantal is nog wel lager dan in 2019, het jaar voor de uitbraak van het coronavirus. Het percentage dat een soa had (20 procent) was lager dan in 2020, maar hoger dan in 2019. Mensen die een melding ontvingen voor een soa, hadden het vaakst infecties. Vanwege de coronacrisis evalueert het RIVM de ontwikkelingen van 2019-2021. Bij CSG's kunnen mensen met een grotere kans op soa, zich gratis laten testen. Sinds augustus 2019 bieden CSG's ook zorg aan mannen die seks hebben met mannen (MSM) en een geneesmiddel krijgen dat hiv voorkomt (Pre-Expositie Profylaxe, PrEP). Deze groep wordt elke drie maanden getest op soa's (MSM in de PrEP-pilot). In 2021 zijn in totaal 138.436 consulten geregistreerd bij de CSG's. Chlamydia In 2021 hadden 20.338 bezoekers aan de CSG's een chlamydia-infectie. Dat is bijna evenveel als in 2019 (21.123). Van het totaal aantal vrouwen dat zich liet testen bij de CSG's had 16,2 procent chlamydia. Van de heteroseksuele mannen was dat 21,3 procent. Deze percentages zijn lager dan in 2020, maar wel hoger dan in 2019. Van de MSM die zich lieten testen had 12,2 procent chlamydia. In 2019 was dat nog 10,6 procent. Onder MSM in de PrEP-pilot is het percentage chlamydia-diagnoses gedaald van 11,7 procent in 2019 naar 10 procent in 2021. Gonorroe Het aantal CSG-bezoekers dat in 2021 gonorroe had (7.842) was bijna hetzelfde als in 2019 (8.180). Van het totaal aantal vrouwen dat zich liet testen, had 1,5 procent deze soa. Van de heteroseksuele mannen was dat 1,8 procent. Deze percentages zijn licht gedaald, na een stijging in 2020, en lager dan in 2019. Het percentage gonorroe onder MSM steeg van 11,6 in 2019 naar 12,4 procent in 2021. Onder MSM in de PrEP-pilot daalde het percentage van 10,7 in 2019 naar 9,2 procent in 2021. Er is geen antibioticaresistentie tegen het huidige 'eerste keus' antibioticum voor gonorroe (ceftriaxon) gemeld. Syfilis In 2021 was het aantal CSG-bezoekers dat syfilis had (1.378) lager dan in 2019 (1.430). Het percentage MSM met syfilis was 2,6 procent in 2021, ongeveer hetzelfde als 2019 (2,5 procent). Onder MSM in de PrEP-pilot is dit percentage licht afgenomen van 2,2 procent in 2019 naar 1,7 procent in 2021. Het aantal diagnoses onder vrouwen en heteroseksuele mannen bleef in 2021 laag, respectievelijk 21 en 33. Hiv In 2021 kregen 128 CSG-bezoekers te horen dat ze hiv hebben, iets meer dan in 2020 (123). Onder hen waren 93 MSM en 17 deelnemers aan de PrEP-pilot. Er waren 6 vrouwen met hiv en 12 heteroseksuele mannen. In 2021, kwamen 794 mensen met hiv voor het eerst naar een hiv-behandelcentrum ('in zorg'). Dat was 18 procent minder dan in 2019 (972). PrEP In de nationale PrEP-pilot hebben 9.782 personen (van wie 97 procent MSM is) een eerste PrEP-consult gehad, van wie 3.450 in 2021. Op 31 december 2021 deden ongeveer 7.693 mensen mee aan de PrEP-pilot. Het percentage deelnemers met een soa was in 2021 17,7 procent.

In 2021, the number of people tested at a Sexual Health Centre (SHC) for a sexually transmitted infection (STI) was higher compared with 2020. This number was still lower compared with 2019, the year before the start of the COVID-19 pandemic. The percentage with an STI (20 per cent) was slightly lower compared with 2020, but higher compared with 2019. In 2021, this percentage was highest among persons notified of an STI. Due to the corona crisis, trends from 2019 to 2021 are evaluated in this report. SHCs offer complimentary STI testing to people at high risk of acquiring an STI. Since August 2019, SHCs have also been providing care to men who have sex with men (MSM) who receive a drug to prevent HIV infection (Pre-Exposure Prophylaxis, PrEP). In 2019, this percentage was 10.6 per cent. Among MSM in the PrEP pilot, the percentage of chlamydia diagnoses decreased from 11.7 per cent in 2019 to 10 per cent in 2021. Gonorrhoea The number of SHC visitors who had gonorrhea in 2021 (7,842) was relatively comparable with 2019 (8,180). Of the total number of women who were tested, 1.5 per cent had this STI. This was 1.8 per cent among heterosexual men. These percentages have decreased slightly, after an increase in 2020, and are lower compared with 2019. The percentage of gonorrhea infections among MSM increased from 11.6 in 2019 to 12.4 per cent in 2021. Among MSM in the PrEP pilot, this percentage decreased from 10.7 in 2019 to 9.2 per cent in 2021. No antibiotic resistance to the current antibiotic of choice for gonorrhea (ceftriaxone) has been reported. Syphilis In 2021, the number of SHC visitors who had syphilis (1,378) was lower compared with 2019 (1,430). The percentage of MSM with syphilis was 2.6 per cent in 2021, comparable with 2019 (2.5 per cent). Among MSM in the PrEP pilot, this percentage decreased slightly from 2.2 per cent in 2019 to 1.7 per cent in 2021. The number of diagnoses among women and heterosexual men remained low in 2021, at 21 and 33, respectively. HIV In 2021, 128 SHC clients received an HIV diagnosis, slightly more than in 2020 (123).
